Genius Acne Scar Treatment
The Genius acne scars treatment is the latest fractional microneedling radio-frequency device that is able to deliver precise energy within the skin. Being a FDA cleared device, the Genius microneedling radiofrequency system is intended for use in dermatological procedures where electrocoagulation and haemostasis is paramount. The Genius treatment differentiates itself from other microneedling RF device by the use of insulated needle and proprietary tissue impedance feedback and control.
Facts on Genius RF Microneedling
How does Genius Acne Scars Work?
The Genius radio-freuquency systems delivers energy with pinpoint precision within the dermis at predetermined depths. Radio-frequency energy is heat energy that is delivered in a fractionated fashion to create multiple micro-coagulation zones. Natural wound healing is induced resulting in collagen stimulation (neocollagenesis), dermal remodelling and acne scar resurfacing over time. Fractional treatments have the advantage of a speedier recovery and lesser risk of complications because more normal skin is able to repair around the treated areas. With the Lutronic Genius acne scars treatment, each treatment is customized to your acne scar type to achieve optimal results.
Practical Improvements of Genius Applied:
- Sharper needle structure with enhancement in tapering, grind angles and smoothness
- High torque motor delivery
- Synchronized tissue impedance monitoring
- More consistent and precise energy delivery at predetermined depths within the dermis
- Accurate and quicker placement of needle tip during your treatment
- Delivers more precise energy with respect to differences in tissue resistance thereby enhancing safety.
Why real time impedance monitoring is important?
What is the ideal temperature for collagen remodelling to occur? We know that treatments with suboptimal heating (<65 degrees celcius) will lead to minimal collagen remodelling. On the other hand, excessive heat (>80 degrees celcius) will lead to tissue dessication and damage. The narrow range of temperature (65-80 degress celcius) is optimal for tissue coagulation and tissue remodelling. The Genius is able to provide real-time feedback on whether too little or too much heat energy is being delivered.
To ensure that only dermal coagulation is taking place, the Genius measures tissue impedance. Basically, impedance is a measure of resistance of electrical flow within the tissue. The Genius is able to constantly monitor the impedance every millisecond, stopping the pulse if the temperature breaches 80 degrees celcius. Without measurement of impedance, less energy is delivered as tissue resistance increases. With real time impedance monitoring, energy delivered is consistent. This feedback mechanism enhances safety during your procedure and reduces the risks of side effects.
Does Genius work for all Acne Scars?
Every individual will have different acne scars. A customised approach taking into account the different acne scar morphology, scar depth, skin condition and treatment goals are integral to achieving a desired outcome. It would be quite uncommon to depend only on one treatment type or device for best results.
For instance, patients with predominantly ice pick scars and atrophic rolling scars will benefit from TCA CROSS . On the other hand, fractional resurfacing techniques such as CO2, INFINI and Genius are more suited for rolling and boxcar scars. Individuals with deeply tethered atrophic acne scars, rolling and keloid scars will probably benefit from subcision, fractional resurfacing and injection steroids respectively.
Genius vs. INFINI vs. Microneedling RF
Since the advent of CO2 fractional resurfacing, there have been advances in technology to deliver energy into the dermis without disruption of the dermo-epidermal junction. The advantages of such a system reduces side effects such as post-inflammatory hyperpigmentation, allowing treatments to be safely carried out in all skin phototypes.
First generation microneedling devices (e.g. dermapen) were rudimentary in that healing depended solely on the microwounds that were produced from the needle. Subsequent microneedling devices incorporated radiofrequency energy to induce coagulation and healing. The continued refinements led to insulated needles, shaper needles, better motor systems and a feedback mechanism to ensure the right energy is being delivered into your skin.
Both Genius and INFINI hail from Lutronic, a company which is focused on outcome based technology. While the Genius can be seen as the ‘upgraded’ version of the INFINI, the results of the latter are by no means inferior. Having the experience of the 2 devices, the key determinant to choosing any acne scar treatment plan begins with a consultation and examination of the scar morphology, scar depth, skin condition and scar grade. A individualised approach that is dynamic and working towards yours goals will lead to better outcomes.
How is the Genius Acne Scars Treatment Carried Out?
On the day of the procedure, your skin is cleansed and a numbing cream applied. Precise energy is delivered within the skin via gold-plated insulated microneedles. The resultant wound healing process triggers more collagen and elastin to form thereby improving the appearance of your scars over time. As the microneedles are insulated, this means that the upper aspect of the skin is protected, and energy is only delivered to where is matters most: the dermis!
The treatment is well tolerated by most individuals with minimal to moderate discomfort levels being experienced.
What downtime can I expect from the Genius Acne Scars Treatment?
There will be temporary redness and mild swelling that may lasts up to 5 -7 days depending on the intensity of the treatment. Occasionally, one may experience transient mild acne. The risks of post-inflammatory hyperpigmentation (PIH) from the procedure is low as most of the energy is released into the deeper dermis instead of the epidermis (upper layer of the skin).
What is the interval between treatments?
Most treatments are spaced between 4-6 weeks. Time is necessary for your skin to heal before your next treatment. On average, 4-6 treatments sessions are recommended depending on your scar type, scar morphology and treatment goals.
Is Genius treatment safe in darker skin types?
The Genius acne scars treatment is a versatile treatment across all ethnicities and skin types. The precise distribution of energy with minimal disruption of the epidermal surface is safer in Fitzpatrick phototype IV to VI. Unlike the fractional CO2 lasers where the incidence of post-inflammatory hyperpigmentation (PIH) is higher, the risk is minimal for Genius acne scars treatment.
What are the other clinical applications of Lutronic Genius system?
The ability of the genius system is versatile and can be used in a variety of settings and body areas. These include:
- Facial acne scars
- Skin rejuvenation
- Skin tightening
- Axillary hyperhidrosis
- Scar treatment (post-operative, traumatic)
The treatment is well tolerated in individuals. We use a specially formulated numbing cream (that is more effective than EMLA cream) prior to your procedure to enhance your comfort.
Healing from the Genius microneedle RF treatment is faster compared to fractional ablative lasers as the top part of the skin is spared during treatment.
Expect some redness and mild swelling after the Genius acne scars procedure. These should fade within 5-7 days. Depending on the intensity of the treatment, you may experience some temporary grid marks that can lasts from 4-8 weeks.
Apply a gentle moisturizer and sunscreen post procedure. It is important to avoid direct intense sun exposure, excessive exercise and alcohol after your procedure.
Generally, expected side effects include transient redness, swelling and mild acne.
Uncommon side effects include:
- Infections – Can be treated with antibiotics/antiviral medication
- Hyperpigmentation – Mostly fade over time
- Persistent redness – More common in sensitive and sun-exposed skin
- Grid marks – Fades within 4-8 wks
- Scarring (rare) – Mostly for herpes/cold sore infection
A consultation is necessary prior to any medical treatments to assess your suitability, discuss treatment options and potential side effects of the treatment offered.
