What is a Fractional CO2 Laser?
The fractional CO2 (carbon dioxide) laser is an effective and proven dermatological laser that offers a wide array of benefits for patients that are seeking to improve their skin appearance and texture. By leveraging on the power and precision of the CO2 laser to deliver controlled microthermal zones (MTZ) to the skin, it induces the body’s healing process to produce new collagen, resurface your skin and improve your skin texture.
OUR APPROACH
Dr Ng's experience with fractional co2 laser
Fractional CO2 laser treatment represents a significant advancement in dermatological and aesthetic procedures. Each fractional CO2 laser treatment is tailored to the individual needs and skin type of the patient, with a comprehensive consultation conducted beforehand to determine the most appropriate treatment plan.
How does the fractional CO2 laser work?
1. Ablative nature:
The fractional CO2 laser delivers precise energy to remove both the epidermis and dermis to a certain depth. Fractional ablative lasers such as the CO2 lasers are generally more effective than fractional non-ablative lasers such as the Erbium glass laser. Unlike the CO2 laser, the non-ablative systems does not target the epidermal component of the skin.
2.Fractional nature:
The unique fractional nature of this laser systems is able to split the laser beam into small beams that are separated by normal skin. The fractional CO2 laser creates tiny, precise columns of thermal damage in the skin, called micothermal zones (MTZ) that is essential for healing, remodeling and regeneration to take place. Compared to fully ablative lasers, the surrounding healthy skin that is interspersed with the ablative ones are able to kickstart the wound healing process that is more rapid and associated with lower downtimes and complications. The depth and intensity of the laser can be adjusted to address specific skin concerns, making it suitable for a wide range of patients.
How does Fractional ablative laser compare to Fractional non-ablative lasers?
Characteristic | Fractional Ablative Laser | Fractional Non-Ablative Laser |
---|---|---|
Type of Laser | CO2, Er:YAG | Erbium glass, Diode |
Mechanism | Removes epidermis and dermis | Targets dermis only |
Depth | Deeper penetration within the skin | Shallower penetration |
Effectiveness | More effective for deeper scars, deeper wrinkles and skin resurfacing | Effective for mild to moderate scars, wrinkles and skin resurfacing |
Side effects | More prolonged redness and higher risk of post-inflammatory hyperpigmentation (PIH) | Faster recovery with lesser risk of PIH |
Ideal candidate | Lighter skin types. Patients with darker skin tones have higher risk of PIH. | Most skin types |
What can the fractional CO2 laser treat?
Acne Scars
The Fractional CO2 laser treatment is FDA-approved and has been extensively studied for its safety and efficacy to treat acne scars.
Surgical/Traumatic/ Burn scars
The Fractional CO2 laser treatment is able to improve skin texture, colour and scar thickness through tissue remodeling and collagen synthesis.
Skin resurfacing
The Fractional CO2 laser effectively treats wrinkles, fine lines, pigmentation, pores and enhances your skin texture.
Skin tightening
The thermal injury stimulates more collagen and elastin fibres. The tissue remodeling leads to enhanced skin texture with a natural lifting effect.
Stretch marks (Striae)
The laser micro-injuries stimulate healing to reduce stretch mark visibility, enhance skin texture and skin elasticity.
Hyperpigmentation
Fractional CO2 laser treatment is a highly effective method for addressing various types of hyperpigmentation concerns like age spots, sun spots and photo-aging.
What to expect during fractional CO2 laser treatment
Pre-Procedure Preparation:
Consultation:
- During the initial consultation, your doctor will assesses the your skin condition, discusses treatment goals, and determines if fractional CO2 laser treatment is appropriate.
- A thorough medical history is taken to identify any contraindications, such as active skin infections.
Skin Assessment and Plan:
- The doctor evaluates the areas to be treated and creates a personalized treatment plan. Photographs may be taken to document the skin’s condition before treatment.
Pre-Treatment Instructions:
- Patients are advised to avoid sun exposure, tanning, and certain skincare products (like retinoids) for a few weeks before the procedure.
- An antiviral medication may be prescribed if the patient has a history of herpes simplex virus to prevent an outbreak.
During the Procedure:
Cleansing:
- The treatment area is thoroughly cleansed to remove any makeup, oils, or impurities.
Topical Anesthesia:
- A topical anaesthetic cream is applied to the treatment area to minimize discomfort. This is typically left on for 30-60 minutes to ensure adequate numbing.
Protective Measures:
- Protective eyewear is provided to both the patient and the provider to shield the eyes from the laser light.
Laser Treatment:
- The provider uses a handheld fractional CO2 laser device to deliver precise beams of light to the skin. The device creates microscopic columns of thermal damage (microthermal zones or MTZs) while leaving surrounding tissue intact.
- The laser settings, including depth and intensity, are adjusted based on the patient’s skin type and treatment goals.
- The doctor makes multiple passes over the treatment area to ensure even coverage.
Cooling and Comfort:
- During the procedure, a cooling device or cold air may be used to enhance patient comfort and minimize heat buildup in the skin.
Post-Procedure Care:
Immediate Aftercare:
- After the procedure, the treated area may be red, swollen, and have a sunburn-like sensation. An ice pack or cooling gel may be applied to soothe the skin.
- A soothing ointment or moisturizer is usually applied to keep the skin hydrated and promote healing.

Frequently Asked Questions
Most patients experience some discomfort during the procedure. To minimize pain, a topical anesthetic is applied to the treatment area before the procedure. Some patients may also receive cooling or cold air during treatment to enhance comfort.
You are not suitable for the fractional CO2 laser treatment if you have the following conditions:
- Active infections
- Abnormal/suspicious mole
- Inflammatory/infective skin conditions
- Exposure to the sun within 4-6 weeks prior of after treatment.
- Tattoos in the treatment area
- Permanent dermal implants
- Pregnancy/breastfeeding

After the procedure, the treated area may be red, swollen, and feel like a sunburn. These side effects typically subside within a few days to a week. The skin may also peel or flake as it heals. It’s essential to follow post-treatment care instructions, including keeping the skin moisturized and protected from the sun.
The number of treatments required varies depending on the skin concern being addressed and the desired outcome. Most patients see significant improvement after one to three sessions. Treatments are typically spaced several weeks apart to allow for proper healing.
The results of fractional CO2 laser treatment can be long-lasting, especially with good skincare practices and sun protection. The skin continues to improve over several months as new collagen forms. Maintenance treatments may be recommended to sustain the results.
Common side effects include redness, swelling, and peeling, which usually resolve within a week. There is a risk of infection, hyperpigmentation, or hypopigmentation, especially in individuals with darker skin tones. Following pre- and post-treatment instructions can help minimize risks.
While fractional CO2 laser can be used on many skin types, it is particularly effective on lighter skin tones. Patients with darker skin may have a higher risk of pigmentation changes and should discuss these risks with their doctor. A patch test or alternative treatments may be recommended for those with darker skin tones.
Post-treatment care includes:
- Keeping the treated area clean and moisturized.
- Avoiding sun exposure and using a broad-spectrum sunscreen with SPF 30 or higher.
- Avoiding harsh skincare products and treatments until the skin has fully healed.
- Avoid environmental irritants during healing process (i.e. dust, dirt, perfume, sun) and stay indoors to avoid direct sunlight.
- Gently wash area with tepid water & mild cleanser 3-4x/day; immediately re-apply a thin layer of occlusive ointment
- Avoid over-sweating until treated area is healed.
The cost of fractional CO2 laser treatment can vary widely depending on several factors, including the indication, the expertise of the provider, the size of the treatment area, and the number of sessions required. Consulting with a qualified provider will give you a more accurate estimate based on your specific needs and treatment goals.
