How does Genius Acne Scars Work?

The Genius radio-freuquency systems delivers energy with pinpoint precision within the dermis at predetermined depths. Radio-frequency energy is heat energy that is delivered in a fractionated fashion to create multiple micro-coagulation zones. The Genius FR Microneedling treatment has been in use since 2019 and we regularly carry out treatments for mainly acne scars resurfacing. The main advanatges of the Genius include:

What Acne Scars Are Most Suited for Genius Treatment?

The approach to acne scars must take into account various factors such your acne scar type, skin type, response to treatment and expected downtime.

Not all acne scars would benefit from Genius RF microneedling.  Deep ice pick scars would benefit from treatments such as TCA CROSS, while deeper rolling scars are better served by subcision and /or fillers. On the other hand, Genius is able to treat acne scars beyond 1.5mm, far beyond most ablative lasers (e.g. CO2 and Erbium) at present.

How is Genius better than fractional lasers?

One of the biggest advantages of the Genius treatment is the ability to treat at greater depths within the skin while sparring the upper layers of the skin (epidermis). Acne scars lies in the deeper dermal layer of the skin. Therefore, a treatment that spares the upper layers while only targeting scars at the deep layer would be ideal.  The specialized insulated needle of the Genius makes treatment not only safer but also more precise.

On the other hand, fractional ablative lasers treat the upper layers as well as the deep layers of the skin. This is associated with increased redness, downtime and potentially more side effects. In addition, fractional ablative lasers do not extend beyond 1mm from the skin surface, limiting treatment to deeper areas.

How is the Genius Acne Scars Treatment Carried Out?

On the day of the procedure, your skin is cleansed and a numbing cream applied. Precise energy is delivered within the skin via gold-plated insulated microneedles. The resultant wound healing process triggers more collagen and elastin to form thereby improving the appearance of your scars over time. As the microneedles are insulated, this means that the upper aspect of the skin is protected, and energy is only delivered to where is matters most: the dermis! 

The treatment is well tolerated by most individuals with minimal to moderate discomfort levels being experienced.

Why real time impedance monitoring is important?

What is the ideal temperature for collagen remodelling to occur? We know that treatments with suboptimal heating (<65 degrees celcius) will lead to minimal collagen remodelling. On the other hand, excessive heat (>80 degrees celcius) will lead to tissue dessication and damage. The narrow range of temperature (65-80 degress celcius) is optimal for tissue coagulation and tissue remodelling. The Genius is able to provide real-time feedback on whether too little or too much heat energy is being delivered. 

To ensure that only dermal coagulation is taking place, the Genius measures tissue impedance. Basically, impedance is a measure of resistance of electrical flow within the tissue. The Genius is able to constantly monitor the impedance every millisecond, stopping the pulse if the temperature breaches 80 degrees celcius. Without measurement of impedance, less energy is delivered as tissue resistance increases. With real time impedance monitoring, energy delivered is consistent. This feedback mechanism enhances safety during your procedure and reduces the risks of side effects.

Is Genius treatment safe in darker skin types?

The Genius acne scars treatment is a versatile treatment across all ethnicities and skin types. The precise distribution of energy with minimal disruption of the epidermal surface is safer in Fitzpatrick phototype IV to VI. Unlike the fractional CO2 lasers where the incidence of post-inflammatory hyperpigmentation (PIH) is higher, the risk is minimal for Genius acne scars treatment.

Skin ageing is due to both intrinsic and extrinsic factors; leading to skin wrinkles, pigmentration, skin laxity and rough skin texture. Radiofrequency microneedling stimulates collagen and elastin, resulting in improved skin tightening, reduced roughness, and more uniform pigmentation. Results are more lasting when compared to botulinum toxin injections. When compared to lasers, microneedle radiofrequency showed better improvement to the nasolabial folds, perioral, jawline and neck wrinkles.

Radiofrequency microneedling is believed to destroy sebaceous glands to reduce its activity and stimulate remodeling of follicular epithelium and dermis. Several studies have shown that it reduces acne severity and prevents longer term acne recurrence. Rosacea is a chronic inflammatory disease of the sebaceous glands that presents as persistent facial erythema, flushing, and occasionally, papulopustular lesions. Improvements in rosacea from radiofrequency microneedling may be due to a reduction in inflammation, blood vessels and dermal remodelling.
