You can dissolve dermal fillers with Hyaluronidase, a soluble protein enzyme that breaks down hyaluronic acid based fillers. The popularity and rise in biodegradable Hyaluronic acid fillers over the years have also seen an increase in filler related complications. Although most complications are mild and easily remediable, there are rare serious complications (e.g. blindness, stroke, and skin necrosis) that necessitates immediate medical attention.

What is Hyaluronidase?

Hyaluronidase was first used in 2004 to remove lumps around the eyes following a HA dermal filler treatment. Hyaluronidase is a life-saver in the field of cosmetic dermatology. It is seen as an ‘eraser’ or ‘reverser’ for the undesirable effects of a hyaluronic acid dermal filler injection. Hyaluronidase breaks down HA dermal fillers by the process of hydrolysis i.e. chemical reaction with water. It is rather specific and does not affect normal skin tissues. 

How does hyaluronidase work?

Glycoaminoglycan, which is the main extracellur matrix of the skin has hyaluronic acid as one of its chief components. This is the same ingredient used in modern day hyaluronic (HA) fillers for aesthetic purposes. Hyaluronidase is an enzyme that breaks down the hyaluronic acid into simpler substances (monosaccharides) which are then naturally  cleared by your body.

Factors affecting that rate of filler dissloution include:

  • Cross-linking: Fillers with more cross-linking will be more difficult to dissolve
  • Concentration: Higher concentration fillers will take a longer time to dissolve.

When Do You Dissolve Dermal Fillers?

You may consider Hyaluronidase to dissolve dermal fillers if there is a bluish discolouration (Tyndall effect due to superficial placement of the filler), lumps and irregularities. Hyaluronidase is also useful to correct asymmetry, overfilling and product misplacement. Vascular occlusion is a rare phenomenon due to the blockage of a blood vessel from the inadvertent injection of the HA filler. There may be pain and purple discolouration of the skin in a case of impending skin necrosis. 

What Types of Hyaluronic Acid Dermal Fillers Are Available?

With the recent popularity of dermal fillers, there are many brands to choose from. They differ in their viscosity, cross-linking and longevity. Common Hyaluronic Acid Fillers are:

How Do You Dissolve Dermal Fillers?

After a clinical assessment, the area is marked out and numbing cream is applied.  Skin testing is recommended for any potential allergy to hyaluronidase on your arm before the injection is administered into the affected area. The amount of hyaluronidase to be injected depends on the size of the area to be corrected. Small lumps and bumps may resolve with 24-48 hours while larger lumps may take up to 2 weeks and require repeated treatments.

are there Any Side Effects?

There may be mild transient discomfort as the hyaluronidase is injected into the affected area. Swelling is mild and resolves in a matter of hours, while bruising can take up to 7 days. Allergic reactions are rare. It is not recommended to go for Hyaluronidase treatment if you had a history of hypersensitivity to bovine collagen, bee stings or have an existing skin infection.
